Following the conclusion of the Coroner’s Inquest, which reached a verdict of misadventure and that the actions of agencies did not cause or contribute to the tragic death of Child L in 2016, Rochdale Borough Safeguarding Children’s Board (RBSCB) has published its Serious Case Review (SCR) into this case.
Jane Booth, Independent Chair of the Rochdale Borough Safeguarding Children’s Board, said: “On behalf of the Rochdale Borough Safeguarding Children Board and its partner agencies I wish to extend our condolences to Child L’s family and friends. The Serious Case review we have published today clearly identifies the lessons that needed to be learned to improve practice when working with vulnerable teenagers and their families. The Board will continue to monitor agency action plans to ensure these improvements are made.”
